Welcome to Care for America’s Workers

Care for America’s Workers coordinate medical care and the staffing needs of injured sick workers through a team approach to delivering healthcare.  Our CFAW Care Coordination Teams are led by Clinical Care Coordinators who are skilled nurses providing on-site coordination of care.  Clinical Care Coordinators communicate with the medical providers to establish treatment plans and to help facilitate optimal functional outcomes.

CFAW participants typically experience an illness—rather than an accident—that results in injury from employment. An exposure to a toxic substance, activities of a repetitious nature, or a traumatic incident are examples of covered benefits and programs we service.

As a CFAW participant, you have 24-hour, 7-day a week access to a member of the Care Coordination Team. Our Team members consist of Registered/Licensed Practical Nurses (Clinical Care Coordinators), Social Workers (Support Coordinators), and Navigators (who assist you in applying, obtaining, and staffing additional assistance you may need in your home). In addition, our Care Coordination Teams work closely with you, your family, caregivers, primary care and ancillary care providers.

We are based right here in your own community.  We work with all non-profit and human service organizations in your area to help meet all of your healthcare needs.  This means we are not here to deny you services, rather meet your healthcare demands.  Although the CFAW team includes highly trained professionals, the most important member of your Care Coordination Team is you!

CFAW professionals work closely with patients and their primary providers (primary providers are typically your primary care physician) to develop a complete health plan. As a new CFAW participant, we will help you in following through with your healthcare plan. It is our mission to assist you in meeting your goals and increasing your quality of life.
